As there is a police investigation underway, we are limited in terms of any comment we can make in relation to the death of Vaughan Te Moananui.

The deceased was receiving outpatient treatment from the Thames community mental health team of the Waikato DHB.

The deceased’s family contacted our Crisis Assessment Team on Saturday morning and the team attended with the police. The police stood our Crisis Assessment Team down.

Our sympathies go out to Vaughan’s family and friends.

Waikato DHB will work closely and assist the Police in their investigation as well as with the subsequent coronial process that will follow.
